Human beings routinely spend a third of their lives lying completely flat and unconscious for eight hours at a time, a sleeping posture that is virtually unknown among our closest biological relatives. Chimpanzees, who share roughly 98 percent of human DNA, have never once in their evolutionary history slept this way; instead, they build woven nests in trees each night and sleep in a semi-reclined, elevated position. Research published in PLOS ONE has shown that great apes deliberately select tree species based on the stiffness and bending strength of the wood, effectively engineering a structurally stable surface before climbing in for the night. This nest-building behavior traces back approximately 23 million years to the Miocene period, when ancient apes grew large enough that sleeping on bare branches became dangerous.

The contrast raises a straightforward question: if the human biological family has slept in engineered, semi-upright structures for 23 million years, why did modern humans decide that a flat mattress was the natural option? The decision is comparatively recent, and the evidence suggests it may not have been a wise one. When early humans descended from the trees, they slept on the ground. The oldest physical evidence of deliberate sleeping arrangements comes from Sibudu Cave in South Africa, where layered grass and plant bedding has been dated to approximately 77,000 years ago.
The leaves were selected in part for their insect-repellent compounds, meaning humanity’s first known sleep innovation was functionally choosing the right leaves to avoid bugs on a cave floor. Lying completely flat on the ground presented obvious problems for ancestors living alongside predators. From a fully horizontal position, humans cannot hear as well, cannot see at all, and cannot spring up as quickly as from a propped or seated posture. Researchers studying the evolution of human sleep posture believe early ground-sleeping humans most likely slept curled on their side or semi-propped against rocks, tree roots, or other people, clustered in groups near fires.
For a single person alone in a quiet room to lie flat and completely unconscious would have been functionally indistinguishable from being dead. Some researchers have suggested this may explain the common experience of waking at 3:00 a. m. with no apparent cause: an ancient survival program checking whether something has found you, a reflex rather than true insomnia.
The first real evidence of a designed sleeping surface with elevation appears in ancient Egypt around 3,400 BCE, where the wealthy slept on raised wooden bed frames. Notably, some of those Egyptian beds were inclined rather than flat, built on a slight downward slope from head to foot with a footboard at the bottom to prevent the sleeper from sliding off during the night. The Metropolitan Museum of Art has documented this design in artifacts from the age of the pyramids. The Egyptians also did not use pillows as they are known today.
They used headrests: flat bases with curved cradles made from stone, wood, or ivory that held the head elevated several centimeters above the sleeping surface. Stone headrests were deliberately chosen because the head was considered the seat of spiritual life and needed protection. The idea of lying completely flat and unsupported apparently did not occur to people who thought carefully about sleep. For most people in ancient Egypt, Rome, Greece, or medieval Europe, none of this mattered, because they did not have beds at all.
The raised wooden bed with a frame was a luxury object. Most people slept on reed mats, straw sacks, or the bare floor of a shared room, sometimes near livestock for warmth, usually next to other people, usually curled or propped up. During the Middle Ages, a furnished bed with a proper frame and mattress was one of the most valuable items a household could own, frequently listed in wills as a major bequest comparable to leaving a car or jewelry today. The average peasant slept on straw on a cold floor, woke multiple times a night, and had never experienced the serene, horizontal mattress sleep that modern humans treat as a natural baseline.
The inner spring mattress, the design around which most current beds are built, was patented by German inventor Heinrich Westphal in 1871. Memory foam was developed in the 1960s by NASA. The modern flat, soft, horizontal sleeping surface has existed in mass-market form for roughly 150 years, a tiny fraction of the 300,000 years Homo sapiens have existed. The health consequences of this modern invention are significant.
Lying completely flat on the back allows gravity to pull the tongue and soft tissues of the throat backward, narrowing the airway. Clinical studies on patients with obstructive sleep apnea have found that sleeping flat on the back produces nearly four times more breathing interruptions per hour than sleeping on the side. More than half of all obstructive sleep apnea cases are classified as positional, meaning the flat posture is specifically the mechanism causing the problem. A study published in The Lancet estimated that roughly 936 million adults worldwide have obstructive sleep apnea.
The primary treatment for the condition is a machine strapped to the face that pumps pressurized air into the throat to compensate for its collapse. The situation has been described as solving a problem that the sleeping position itself created. The Hadza of Tanzania, one of the last hunter-gatherer groups studied for their actual sleep patterns by researchers at UCLA and published in Current Biology, sleep about six and a half hours on the ground in small grass huts near fires, without mattresses or sleep infrastructure. They have strong circadian rhythms, wake feeling rested, and do not appear to have a sleep apnea epidemic or a mattress industry.
The evidence from 23 million years of ape evolution, 300,000 years of human history, and surviving populations still sleeping closest to the original model suggests the human body was shaped for a semi-reclined or side-lying position on a surface that is neither perfectly flat nor perfectly soft, near other people, with the upper body slightly elevated. Modern humans have a platform mattress, a temperature-controlled room, and sleep supplements, yet still describe their sleep quality as mediocre. Their ancestors wove tree branches into bowl shapes every night simply to rest without dying, and they survived.
